Presenting Problem and History of Problem

The client is a new student a Milton Hershey School and is having issues adjusting to the lifestyle that Milton Hershey offers. He is not used to the structured life that Milton Hershey School mandates and he feels that the classes he is taking are not interesting or challenging. He is currently working on re-establishing a relationship with his mother who has recently been released from jail, and would like to be under her guardianship again.

 

Background Information

  The client is a 16 year old male of Hispanic descent. He is currently a student at Milton Hershey School, where he has been attending since January of 2011. The client lived with his parents until the age of 12, when his parents were sent to jail for drug related charges. At this time, his grandmother was given custody of him and his sister. During the period of time that he lived with his parents, the client witnessed his mother hit his father and both of his parents participate in drug-related activities. He was homeless for the first year that he lived with his grandma, transferring from missions to temporary apartments and motels. 

The client was sent to Milton Hershey School because his grandmother was struggling from a number of mental health issues, including Schizophrenia. She does not speak English and found it stressful to take care of both the client and his sister, due to the issues the client’s sister faces. The client’s sister is in the 3rd grade and suffers from both ADHD and Schizophrenia. She had a suicide attempt recently and remains living with her grandmother. 

The client’s parents are both recently out of jail. The client has no relationship with his father, but is working with the social worker to begin the process of getting his mother as his guardian.

 

Educational and Work History

The client is a sophomore at Milton Hershey School. The guidance counselor from his previous school described the client as a hard-working student. He always participated in class and received B-level grades. Currently, the client has aspirations to go to college. He has a few schools picked out and is excited to begin applying. The client says that he takes his education seriously and would like to eventually become an architect. He finds it hard to pay attention in class because he does not see how his classes relate to his future, and is eager to be out of school.

 

Mental Status

According the WASI intellectual assessment, the client has an average intellectual ability. He also scored proficient in both the reading and math portions of the Pennsylvania System of School Assessment. The client appears to think clearly and is goal oriented. He also exhibits good judgment in that he attends Milton Hershey School voluntarily. He has expressed that he does not want to end up like his parents and feels that Milton Hershey School is a good fit for him because it keeps him off the streets and away from drugs. This shows that the client really wants to succeed. 

 

Impressions and Recommendations

The client appeared to practice good hygiene and to be in good spirits. He seems to be open and inviting, with no problem making friends. I think that he has overcome many obstacles in his life, and he has not always had good role models to look up to. Despite of this, the client has the right mind set to get himself where he wants to be and I have no doubt that he will succeed in doing so.
