Individualized Education Plan (IEP) 1c
Through my Special Education Block in college, I worked with five other students to create and complete a full Individualized Education Plan based off a case study provided by our professor. Fulfilling the role of a Transition Specialist, I completed additional assessments pertaining to my role, and provided an evaluation report on the student as well. The conclusion of our project ended with a meeting with all of the specialists and teachers to determine and finalize the needs of our student. I feel this artifact fulfills the domain under a variety of requirements for Planning and Preparation, such as knowledge of students, knowledge of resources, designing assessment, and especially setting instructional goals.
Below is a short Evaluation Report for my professor assuming my role as the Transition Specialist in the case (pages 1-6). Pages 7 to 11 highlight some of the pages written in our IEP within my role. I also have a paper copy of the entire IEP available.
